Transition Coordinator

1. Transition Coordinator will service all member districts; travel is expected
2. Transition Coordinator will be housed in the SKACD Central Office (Dodge City) unless otherwise agreed by the Executive Director
3. Assist with facilitation of end-of-year transition meetings at each of the following locations:
   1. Elementary to middle school
   2. Middle schools to high school
   3. High school to adult services
4. Work closely with all secondary programs and adult services agencies across member districts and catchment areas
5. Train all secondary special education teachers about appropriate transition planning methods
6. Assist teachers in locating community programs and classroom resources to assist students in meeting functional and adaptive needs upon their graduation
7. Develop strong partnerships with outside agencies (CDDOs, KDADS, SRS, AWI, Compass, etc)
8. Provide inservice training; some inservice to be provided during the summer
9. Attend individualized education meetings (as necessary)
10. Provides procedural oversight on Indicator 13 and Indicator 14
11. Coordinate with case managers, school counselors, and administrators at each member district to ensure that each student’s special education services are being provided
12. Evaluate students’ vocational and rehabilitation needs and/or develop goals in these areas (as needed)
13. Complete and submit all required documentation in a timely manner
14. Other duties as assigned 
15. 10 month contract likely / possibility of 11 month contract

Health insurance (single person) provided. 403(b) matching plan: up to $100/month. If you are not currently certified/endorsed in your area of special education, we will pay 100% of your master’s program to enable you to become certified. $10,000 towards student loan repayment; we will repay up to $1,000 per semester. This includes student loans you accrued prior to working for SKACD. Base starting salary of $41,200. Exact salary is determined by computations according to the 2020-2021 salary schedule, available at www.skacd.com
